Candidates in the High-tech Zone:

The 2021 nurse qualification certificate of the High-tech Zone will be issued from July 27, 2021. The relevant matters are hereby notified as follows:

1. Pick-up time: July 27th-August 13th, 10:00-12:00 in the morning, 14:00-16:00 in the afternoon (except weekends and holidays).

2. Pick-up address: Chengdu High-tech Medical Association 204 Office (No. 65 Xinyi West Street).

3. Receiving requirements:

1. Collect by yourself: Please bring your original ID card;

2. Collecting on behalf of: issuing the authorization letter of the certificate holder and the original ID card, and the original and copy of the ID card of the representative;

3. Unit collection: Bring the unit's letter of introduction (with official seal), collection list (indicating name and ID number), and the original and copy of the recipient's ID card.
