1. Pick-up time: August 19-August 31, 9:00-12:00 in the morning, 13:30-17:00 in the afternoon (except weekends and holidays).

2. Pick-up address: Chengdu High-tech Medical Association 209 Office (No. 65 Xinyi West Street).

3. Receiving requirements:

   1. Collect by yourself: Please bring your original ID card;

   2. Collecting on behalf of: issuing the authorization letter of the certificate holder and a copy of his ID card, and the original ID of the person collecting the certificate;

   3. Unit collection: Bring the unit's letter of introduction (with official seal), collection list (indicating name and ID number) and the original ID of the recipient.
