Chronic diseases such as hypertension, hyperlipidemia and diabetes seriously threaten human health. Comprehensive management of chronic diseases requires collaborative management between tertiary hospitals and grassroots hospitals. Since 2013, China has also accelerated the construction of a hierarchical diagnosis and treatment system centered on medical consortia. The establishment of a medical consortium can not only alleviate the problem of difficult medical treatment, but also improve the chronic disease management skills and level of grassroots general practitioners and family doctor teams. It is an important prerequisite for improving China's chronic disease management rate and compliance rate, and also a new path suitable for collaborative development of medical institutions at all levels in the region.

The third phase of the "co management of chronic diseases, from" sugar "planning" tour program organized by Chengdu High tech Medical Association was successfully held on April 25, 2024. It deeply discussed the comprehensive management strategy of chronic diseases under the hierarchical diagnosis and treatment system, especially the strategy of comprehensive management of diabetes, created a high-level academic exchange platform for diabetes researchers, clinicians, research institutions and related enterprises engaged in chronic disease research, and promoted academic exchanges and transformation of scientific research achievements in related fields.
