Chronic diseases such as hypertension, hyperlipidemia and diabetes seriously threaten human health. Comprehensive management of chronic diseases requires collaborative management between tertiary hospitals and grassroots hospitals. Since 2013, China has been accelerating the construction of a hierarchical diagnosis and treatment system with medical consortia as the core. The establishment of medical consortia can not only alleviate the problem of difficult medical treatment, but also improve the chronic disease management skills and level of grassroots general practitioners and family doctor teams. It is an important prerequisite for improving China's chronic disease management rate and compliance rate, and also a new path suitable for the coordinated development of medical institutions at all levels in the region.
