Editor's note

At 1 am, a seriously injured 48-year-old man was rushed to the emergency department of West China Hospital of Sichuan University. The scene shocked everyone: a steel bar about 1 meter long was inserted from the right neck of the man, throughout his chest, and out from his left underarm. "It is a miracle that [he] survived," said Dr. Huaxi, who is used to all kinds of dangerous foreign body injuries. The miracle continues. Liao Hu, deputy director of the thoracic surgery Department of West China Hospital of Sichuan University (Chairman of the Thoracic Surgery Committee of Chengdu High-tech Medical Association) team successfully carried out the operation. The reporter learned that after emergency surgery and about 1 month of treatment, the man recovered and was discharged from hospital recently.

Miracle! Rebar through the man "around" open vital organs

In the early morning of March 24, Liao Hu, deputy director of the thoracic surgery department of the West China Hospital of Sichuan University, received a phone call from the hospital, "It is said that the emergency department came to a patient with chest cavity injury." Hang up the phone, Liao Hu hurried to see, "Frankly, after seeing patients, I am a feeling of despair."

The patient, a 48-year-old man, had fallen from a 3-meter-high building in Kangding City about six hours earlier and was driven by a 1-meter-long steel bar through his neck, through his chest, and out through his left underarm.

As a doctor, Liao Hu knows what it means when a steel bar runs through the chest. "Such a thick steel bar (2.6 x 1.9cm in cross section) penetrating through these areas could have damaged any organ in the chest with little chance of survival."

At the time of arrival, the critically injured man was less conscious but his vital signs were stable. Liao Hu and the medical team urgently completed the preoperative examination for the man and held an emergency consultation. Amazingly, this rebar seems to have miraculously avoided all vital organs. Liao Hu introduced, "The area where the steel bar 'walks' avoids the subclavian artery, the common carotid artery and other large blood vessels, avoids the aorta, and avoids all vital organs." Although the steel bar went directly through the middle of the lung, it missed the important blood vessels in the lung and the bleeding was not very severe."

"This is the silver lining of misfortune!" The doctors sighed.

Although the rebar appears to have avoided all vital organs, according to the medical imaging, the medical imaging is only auxiliary and requires immediate thoracotomy to explore.

Multi-disciplinary escort, the man recovered after surgery discharged

This is a very tricky operation. Liao Hu and his colleagues in the department of thoracic surgery communicated repeatedly with the departments of otorhinolaryngology-head and neck surgery, vascular surgery, orthopedics, radiology and other departments to make sure nothing was wrong. During the operation, professors from relevant departments participate in the on-stage consultation in the operating room together, and they can immediately come to the stage to assist in any relevant emergency situation.

At 6 o 'clock in the morning, Liao Hu and Hu Yang, deputy chief physician and Shen Cheng, attending physician of the Department of Thoracic Surgery, under the escort of multiple disciplines, emergency thoracotomy exploration, removal of foreign bodies in the chest, lung wedge resection and rupture repair, neck destitching and other operations were performed. In the process, the doctors found that the steel bar did miraculously avoid important nerves and blood vessels, and the process of removing the steel bar did not hurt the aorta and the heart, avoiding massive bleeding, and protecting the safety of the patient as much as possible.

After surgery, the injured man was taken to the intensive care unit and transferred to the thoracic surgery ward two weeks later as his condition was stable. The doctor said that after reviewing the chest CT, no obvious abnormalities were found and there were no serious complications. The vital signs of the injured man were stable and his self-care ability recovered. Recently, the man recovered and was discharged from hospital.

According to reports, although not a rare case, but this patient's special is that the threaded steel bar is long and thick, through the neck through the left underarm and then through the entire chest. The thoracic surgeon of West China Hospital reminds: When there is an accident around, when there is a foreign body throughout the body, do not remove it by yourself, otherwise it is easy to cause secondary damage, and the patient needs to be sent to a professional hospital for timely emergency treatment. In addition, in industrial and agricultural production, traffic accidents, there are still some external foreign body injuries, it is also recommended not to pull out, especially the left forechest foreign body injury, it may hurt the heart, large blood vessels and other important organs. If you pull it out on your own, the patient can bleed out uncontrollably and die instantly.
